Prolonged clotting time after CPB: heparin effect or factor deficiency?

After CPB, prolonged clotting time can reflect residual heparin, factor deficiency, dilution, hypothermia, sampling timing, or protamine effect. The key is not simply whether to give more protamine.

Clinical scenario

A 74-year-old woman undergoes mitral valve repair with CPB. After protamine, bleeding remains diffuse. ACT is only mildly prolonged compared with baseline. ROTEM shows prolonged clotting time in a heparin-sensitive channel, but the heparin-neutralized comparison is closer to normal. FIBTEM clot firmness is acceptable. Platelet count is 110,000/µL. Temperature is 35.8°C and pH is 7.31.

The team asks whether this is factor deficiency or residual heparin.

Reasoning goals

  • Recognize that post-CPB prolonged clotting time is timing-dependent.
  • Separate residual heparin effect from factor deficiency when heparin-sensitive and heparin-neutralized comparisons are available.
  • Avoid treating clotting time prolongation as one diagnosis.
  • Consider both protamine under-reversal and protamine excess.
  • Correct physiology and reassess the bleeding.
  • Keep surgical bleeding in the differential.

A. Why CPB timing matters

  • CPB-in-progress, pre-protamine, post-protamine, and post-CPB bleeding are different clinical situations.
  • Heparin exposure and reversal change how the VET pattern is interpreted.

B. What does the pattern suggest?

  • Heparin-sensitive prolongation with improvement on the heparin-neutralized comparison suggests a residual heparin effect may be contributing.
  • Acceptable fibrin-based clot firmness makes fibrinogen less likely as the dominant problem.
  • Platelet contribution and the surgical field still matter.

  • Factor deficiency or dilution
  • Hypothermia
  • Acidosis
  • Sampling timing
  • Protamine effect
  • Ongoing bleeding

D. Protamine is not “more is always better”

  • Under-reversal can leave a residual heparin effect.
  • Excessive protamine can contribute to anticoagulant-like effects and platelet dysfunction.
  • Additional protamine should be considered only when the pattern, ACT and heparin-sensitive data, timing, and institutional practice support it.

A prolonged clotting time is a pattern, not a diagnosis

Residual heparin and factor deficiency can produce a similar prolongation. The heparin-sensitive comparison, timing, and bleeding pattern are what separate them — the number alone does not.

  • Compare heparin-sensitive and heparin-neutralized channels.
  • Review the ACT trend and the clinical bleeding.
